The Perfect Patient Pathway was an awarded Wave 1 NHS England Test Bed site which was established in 2016. It involved 28 partners spanning across health, social care, education, technology and voluntary sector organisations in the Sheffield City Region.

We aimed to create the perfect patient pathway to bring substantial benefits for patients suffering from long term health conditions, such as diabetes, mental health problems, respiratory disease, hypertension and other chronic conditions.

The Wave 1 test bed offered new solutions to keep patients with long term conditions well, independent and avoiding crisis points which often resulted in hospital admission, intensive rehabilitation and a high level of social care support.

Our team are currently leading on the delivery of the Digital Care Home project. There are still opportunities for regional partners to get involved. To find out more information, please visit our project page.
